ES1370 driver for BeOS R4

This is a quick recompilation and adaptation to the R4 of HockYiung Huang's driver for the ES1370 audio chip. It has been tested with success on a Sound Blaster PCI 128 and a SB PCI 64.

THIS IS FOR R4 ONLY. It doesn't work on R4.5 and R5 and should not be needed with these versions since the ES1370 chip is now supported by Be.
